Accidents happen – and afterwards, it’s usually clear what went wrong. Or is it?
In reality, it’s often not that simple. When we try to learn from accidents, invisible obstacles stand in our way: our own cognitive biases.
Heuristics and Biases
– mental shortcuts and distorted perceptions – help us make quick decisions in everyday life. However, when learning from accidents, they often lead us astray.
Four cognitive biases play a particularly significant role:
Fundamental Attribution Bias: “They were simply negligent”
After an accident, our first impulse is often to blame the people directly involved.
“They should have been more careful!”
What we overlook: decisions are never made in a vacuum. Time pressure, poor communication, unsafe conditions – all of these influence behavior.
If we only focus on individual misconduct, we miss the opportunity to improve the system. And that is precisely what sustainable safety requires.
False Uniqueness Bias: “That could never happen to us”
When an accident happens elsewhere, many organizations think:
“Well, they made mistakes – things are better here.”
This is the False Uniqueness Bias: we believe we are smarter, better prepared, less vulnerable.
But precisely this attitude prevents learning.
Instead of benefiting from the experiences of others, we lull ourselves into a false sense of security.
Yet the crucial question would be: “Where might we be just as vulnerable?”
Confirmation Bias: “Everything points to human error”
We love to be right. Therefore, we automatically look for evidence that confirms our initial assumptions and ignore the rest.
In the context of accident investigations, this means: once the hypothesis of “human error” is on the table, all facts are viewed through this lens.
The problem: if we don’t also take contradictory evidence seriously, important insights are lost.
Good learning requires openness – and the willingness to question one’s own assumptions.
Hindsight Bias: “Everyone should have seen that coming!”
After an accident, everything suddenly seems obvious:
“Why did no one realize that this was bound to go wrong?”
This is where hindsight bias strikes. With today’s knowledge, it seems crystal clear to us what had to happen back then.
Yet, in the actual situation, the circumstances were often complex, confusing, and marked by many uncertainties.
Sidney Dekker’s tunnel example perfectly illustrates this:
After a fatal tunnel collapse, everyone thought it was completely irresponsible to allow work there.
However, the workers themselves felt pressured, there were no acute warning signs – and the decision seemed perfectly reasonable at the time.
If we ignore this, we miss the opportunity to recognize true learning chances: namely, to understand why well-trained people in complex systems sometimes produce poor results.
Conclusion
Heuristics and biases are normal – we all fall into these traps.
It is important that we recognize them and consciously counteract them:
- Instead of seeking culprits: Understand the context.
- Instead of “it won’t happen to us”: Honestly examine our own weaknesses.
- Instead of premature hypotheses: Remain open to complex explanations.
- Instead of being wise in hindsight: Ask how the situation truly presented itself at the time of the event.
Only then do we turn accidents into genuine opportunities to improve.
